How do I make a new patient appointment?Please call 206-315-7998, email or contact us here. We look forward to speaking with you to set up your new patient appointment!

What should I wear to my appointment?Great question! Please wear or bring extra clothes that are comfortable and/or athletic.

How should I prepare for my first appointment?

  • Fill out your new patient paperwork
  • Wear comfortable clothes
  • Bring your ID and insurance card if applicable
  • Arrive 5-10 minutes early
  • Bring imaging, reports etc. that might be applicable to your injury
  • Workers Comp and Car Accidents- Please bring any reports or other information pertaining to your case.

How long is the initial appointment?You should expect your initial appointment to last between 45-60 minutes. This will involve an exam and treatment.

Where are you located?Our address is 4915 25th Ave NE, Ste. 104, Seattle, WA 98105. We are located across the street from University Village Shopping Center on floor 1 of the building. If headed North on 25th Ave, turn left into the driveway just past the Chase bank.

Where should I park?Our building has free underground parking as well as bicycle racks. We are also conveniently located near bus stops along 25th Ave.

How do I know what my insurance will cover?Great question. We strongly suggest that you call your insurance carrier to determine what will be covered. Unfortunately, we do not have access to your detailed insurance information and require the patient to determine their individual benefits. When speaking with your insurance representative, please ask about both chiropractic and physical therapy benefits as we provide both services.

Each plan is different and has unique requirements. Please call us if you have questions. If you are paying without insurance, great! Your life just got a lot easier, and you will just pay our fee schedule at the time of service.

I don’t see my insurance plan listed and/or I don’t have insurance. What can I do? Don’t worry! We have a fee schedule that we use to bill you directly if you do not have insurance or if your carrier is not listed! If you pay at the time of your appointment, we offer a discount on those fees. Call us and we can give you more details!
